Execution

  1. 1Perform the relocation / Jobe relocation test and record whether the relevant labral / instability symptom response is positive.
  2. 2Perform O’Brien active compression testing and record deep shoulder pain or painful clicking that improves in the palm-up position.
  3. 3Interpret the cluster as positive only when both relocation and O’Brien findings are positive.
  4. 4Avoid counting AC-joint-localized O’Brien pain as a SLAP / labral positive.
  5. 5Integrate the cluster with history, mechanical symptoms, instability history, and imaging when required.

Positive outcome

Both relocation test and active compression / O’Brien findings are positive. In Magee’s eAppendix this combination is reported as a labral-tear cluster, not an isolated SLAP-only rule.

CommentMagee’s listed cluster is for labral tear broadly: relocation + active compression, LR+ 4.56. It is not a pure SLAP-only cluster and should be labelled carefully in the app. Use it as moderate supporting evidence when symptoms and mechanism fit a labral lesion.
