Execution
- 1Position the patient side lying with the test arm uppermost.
- 2Stabilize the scapula and clavicle with one hand.
- 3Hold the patient’s arm at the elbow in approximately 30° abduction.
- 4Externally rotate the shoulder while pushing the arm proximally and extending it through the elbow / upper arm.
- 5Note pain or clicking in the glenohumeral joint.
Positive outcome
A click or pain in the glenohumeral joint is positive for a superior labral / SLAP tear. The test aims to compress the superior labrum against the glenoid.

CommentThe passive compression test has promising values from its original diagnostic study, but it is not as widely validated as many common shoulder tests. Different labral lesions and coexisting cuff pathology can alter accuracy. Consider it supportive rather than definitive.
