Execution
- 1Position the patient supine with the legs extended and pelvis square.
- 2Ask the patient to flex both hips and knees so the feet rest flat on the table.
- 3Lift the pelvis slightly or asks the patient to bridge, then lowers the pelvis back to the table.
- 4Extend both legs passively and gently until the heels rest on the table.
- 5Compare the position of the medial malleoli and note any apparent leg length asymmetry.
Positive outcome
A difference in the level of the medial malleoli after the manoeuvre suggests a functional leg length difference. If the apparent difference changes compared with true leg length measurement, pelvic or hip positional factors are suspected. The test does not identify the exact source of the asymmetry.
CommentMagee lists the Weber-Barstow manoeuvre under leg length testing for the hip. It is useful as a quick screen for functional asymmetry, but it is sensitive to patient positioning, pelvic rotation, knee flexion contracture, and examiner alignment. Confirm meaningful findings with true leg length measurement and gait observation.
