Execution

  1. 1Position the finger so the nail bed is visible.
  2. 2Compress the nail bed until the color blanches.
  3. 3Release the pressure.
  4. 4Time how long it takes for normal color to return to the nail bed.
  5. 5Compare the finding with other digits or the opposite hand.

Positive outcome

Color return taking longer than 3 seconds is abnormal and suggests arterial insufficiency to the finger. Magee notes that comparison with the normal side helps judge restricted flow. This is a screening sign, not a diagnosis of the cause of vascular compromise.

CommentDigital blood flow testing is quick and useful after trauma, swelling, splinting, or suspected vascular compromise. Ambient temperature, nail polish, lighting, and systemic perfusion can affect interpretation. Escalate if delayed refill is accompanied by pain, pallor, cyanosis, numbness, or coldness.
