Execution

  1. 1Position the patient supine.
  2. 2Cup the heel and passively flexes the patient's knee fully.
  3. 3Allow the knee to passively extend while supporting the heel.
  4. 4Observe whether the knee reaches full extension smoothly.
  5. 5Note end-range pain, rubbery block, or springy resistance.

Positive outcome

A rubbery block, rebound, or inability to reach full passive extension is positive. The sign suggests intra-articular pathology such as meniscal tear, loose body, or effusion-related block. It does not identify the exact structure.
