Execution

  1. 1Position the athlete supine with knees flexed or straight according to the chosen protocol.
  2. 2Ask the athlete to curl the trunk diagonally toward the opposite knee or shoulder direction.
  3. 3Apply resistance to the trunk or shoulders to load the abdominal obliques.
  4. 4Repeat to both sides for comparison.
  5. 5Record pain location, weakness, asymmetry, and whether symptoms match the athlete’s complaint.

Positive outcome

Reproduction of familiar lower abdominal, pubic, or inguinal pain during resisted diagonal trunk flexion is positive. Pain on one direction may implicate the oblique or rectus-adductor aponeurotic complex on the loaded side. The result should be interpreted within the Doha groin pain classification.

CommentThis is a sport-specific provocation test for suspected core muscle injury and athletic pubalgia patterns. It has limited single-test diagnostic evidence and should be paired with palpation, resisted adduction, hip testing, and imaging when symptoms persist. Value is low because structural specificity is limited.
