Pepper Test

Lateral Elbow Pain

Execution

  1. 1Position the patient’s elbow in approximately 90° of flexion.
  2. 2Place the thumb on the radial head or posterior radiocapitellar region.
  3. 3Maintain palpation pressure over the radiocapitellar joint.
  4. 4Passively extend the elbow while maintaining pressure.
  5. 5Record posterior or lateral radiocapitellar pain reproduction.

Positive outcome

Reproduction of posterior or lateral elbow pain during radiocapitellar palpation with extension is positive. The test is intended to suggest intra-articular radiocapitellar pathology or minor instability in recalcitrant lateral elbow pain.

CommentThis test was not found in the main Magee elbow text and comes from Arrigoni’s SALT / PEPPER pilot work. The study focused on recalcitrant cases already diagnosed with lateral epicondylitis and compared tests with arthroscopic findings. Because prevalence was high and the sample was small, positive predictive values should not be treated as stable diagnostic performance.
