Execution
- 1Flex the patient’s elbow to approximately 60° to 80°.
- 2Support the forearm in a relaxed position.
- 3Squeeze the biceps muscle belly near the elbow.
- 4Observe whether the forearm supinates.
- 5Compare the response with the opposite side when needed.
Positive outcome
Absence of forearm supination during biceps squeeze is positive for distal biceps tendon rupture. Supination response suggests an intact distal biceps mechanism.

CommentMagee’s eAppendix reports 96% sensitivity and 100% specificity, yielding a very strong rule-in profile. The 100% specificity and infinite LR+ still come from a limited clinical sample, so the exact multiplier should not be overread. In practice this is a useful confirmatory sign when combined with history and hook / BCI findings.
